Birth Certificate Information
The Florida Department of Health can issue birth certificates for people born in Florida from 1930 to the present. Birth certificates for people born in Florida in the years prior to 1930 must be obtained from the State Office of Vital Statistics in Jacksonville, Florida.
Birth certificates are needed For:
Admission to public school
Getting a driver's license
Work permits
Registering to vote
Eligibility for retirement
Social Security benefits
Proving citizenship
Obtaining public assistance for dependent children
Birth certificates can be issued to:
Parents
Individual 18 years of age or older, with proper identification
Legal guardians
Fees:
Birth Certificate $15, additional copies are $10

The Vital Statistics Office in Lecanto is open Monday through Friday from 8:00am-4:30pm.
Include the following information:
A photocopy of your driver's license for identification
Name of child
Date of birth and county of birth
Mother's maiden name and father's name
Relationship to the person on the birth certificate
