- Travel itinerary
- Previous shot record
- International Certificate of Vaccination (Yellow Book)
- $100 payment for the consultation visit for each member of your travel group

Some Things to Note
- Payment is required prior to service
- Total cost will include the $100 consultation fee, cost of service and administrative fees
- You will be advised of the estimated total cost at your consultation visit
- Insurance will not be billed. Each individual will receive a receipt for services which they can submit to their insurance company for possible reimbursement.
- If you are traveling in a group, each person in your party will need their own consultation appointment.
- Please arrive at least 30 minutes prior to your appointment so that you will be able to complete any paperwork.
- Each traveler will be required to sign a form stating whether they accept or decline services required or recommended for his or her travel destinations.
- Travelers over age 60 who are visiting areas where Yellow Fever is a concern must present a doctor’s note or prescription stating that they are “immune competent to receive” any preventative services.
- Some preventative services will be administered as a series and require multiple visits. An administration fee is required for each service or visit.
Since some services require several weeks to become effective, it’s important to schedule an appointment accordingly. For assistance, contact DOH-Citrus at 352-513-6090.
